1. I will live without violating any law. A conviction in a court of law is not necessary for such a violation of law to constitute a violation of my probation, community control, or any other form of court ordered supervision.

2. I will report all arrests, including traffic violation/citations, within 24 hours regardless of the outcome to my Probation Officer.

​

3. I will not possess, carry, or own any firearm. I will not possess, carry, or own any weapon without first procuring the consent of my Probation Officer.

​

4. I will work at a lawful occupation and support my dependents, if any, to the best of my ability.

​

5. I will keep my Probation Officer informed of my current phone number and current address at all times while on supervision.

​

6. I will promptly and truthfully answer all inquiries directed to me by the Court or my Probation Officer; will allow my Probation Officer to visit in my home, at my employment site or elsewhere, and I will comply with all instructions my officer may give me. I will obey all oral and written instructions of my Probation Officer and abide by any sanction imposed upon me as a result of an alleged violation of the terms and conditions of my supervision.

7. I will provide any documentation or verification requested by my Probation Officer as soon as possible.

​

8. I will not use intoxicants (beer, whiskey, wine, etc.) of any kind to excess, or use or have in my possession illegal or non-prescribed drugs or drug paraphernalia. I will submit to drug screens as required during my time on supervision, including oral, urine, dermal and, at my expense, blood. I understand that failure to comply is an admission of guilt.

9. I agree to a search, without warrant, of my person, vehicle, property, or place of residence by any Probation Officer or Law Enforcement Officer, at any time.

​

10. I agree to pay all required court costs/fines/fees/restitution to the sentencing court as ordered by the Judge. I agree to pay all imposed fees/costs necessary for and during my supervision to the probation office, unless waived by appropriate authorities.

12. I will not engage in any assaultive, abusive, threatening, or intimidating behavior. I will not behave in a manner that poses a threat to others or myself. I will not participate in any criminal gang related activities as defined by TCA 40-35-121.

13. I will complete ___________ hours of community service work during my time on probation, to be performed at the instruction of my Probation Officer or the Court.

​

14. I will not abscond from supervision. I understand that avoiding service of process, arrest, or prosecution, refusing to report as instructed, leave the approved area of travel or actively avoiding Probation Officers are all acts of absconding.
