What you need to know about Intake:
​What to bring:
​You MUST bring a credit/debit card or money order for payment of your 1st month's supervision fees, drug screen fee and intake fee. If you do not bring the required payment, you will not be able to complete intake and will be rescheduled once. If you continue to fail to bring payment, your probation sentence may be violated through the court.
The monthly fee is $45 per month unless you are receiving Social Security benefits then the amount each month is $35. You must bring a benefit letter for proof of receiving that amount each month. (NOTE: If you are filing for disability but are not receiving those funds currently, we will not be able to reduce your monthly fee. You MUST be receiving those benefits to have the fees reduced. We DO NOT waive the fee entirely, it is only reduced.
The drug screen fee is $20 for each cup. If the sample must be sent to the lab for confirmation, there is an additional fee of $55 to have that service completed.
The intake fee is $20 and due on the date you are set for an intake appointment.​
Bring any employment verification information if you have not submitted it in the online form.
What to know:
You MUST be ready to provide a urine sample for a drug screen. If you cannot provide a sample you will have up to 2 hours to provide a sample. If you cannot provide a sample at that time, it will be the discretion of the intake officer if you will be allowed additional time.
If you are taking any prescription drugs, you MUST provide a printout from your pharmacy showing the medications being taken. You may also bring the receipts from the prescription bags, if you cannot get a printout from your pharmacy. Any substance that shows a positive result without the proper prescription verification will be marked as a positive drug screen and may result in sanctioning.
If you refuse to submit to a drug screen within the given time frame, it will be a violation of your probation sentence.
You will be in intake for approximately 1-2 hours so plan accordingly. If you are depending on a ride for your appointment, let him/her know this is the amount of time you will be at our office.
If you cannot complete the required forms online, please be sure you do so when you arrive for intake. All paperwork must be completely filled out and sign each form as needed.
You will have a photo taken for your file at intake.
When you arrive at our office for intake, please enter the building in the basement NOT the lobby. Intake appointments are completed downstairs. The doors will be opened 15 minutes prior to your appointment.
